The above providers are NOT preferred providers for ANY INSURANCE COMPANY.
We ONLY accept assignment on Medicare.
We DO NOT accept MEDI-CAL or ANY HMOs. Patients are responsible for any charges incurred at the time of their visit and will be responsible for payment at the time of their visit.
Please be advised that we still ACCEPT INSURANCE and will bill your insurance company as a courtesy to you, but you will be billed for any amount your insurance doesn’t cover. Pleas note that ALL out-of-network co-insurance, deductibles and co-pays are YOUR RESPONSIBILITY. Your out-of-network benefits apply to these services:
- Audiology (Hearing Tests)
- Operating Room Services

**PLEASE NOTE: Dr. Griffiths, Dr. Lee, and Dr. Putnam patients who have BLUE SHIELD OF CALIFORNIA, we will now have to collect $200 at the time of service, as BLUE SHIELD will send the payment directly to you.

NON-MEDICARE PATIENTS: Please note: we are out of network for ALL INSURANCE COMPANIES and you will be responsible for payment at the time of your visit. The amount will depend on the treatment you receive but will be between $190-$580. We will provide a copy of your bill for you to submit to your insurance company for reimbursement.
MEDICARE PATIENTS: If you have
MEDICARE, we will bill this for you and you will only be responsible for a refraction fee of $50, if we provide this service to you as well as deductibles or co-pays, if not covered by your secondary insurance.
